New Patient Information
We at CCC are proud to provide you with highly qualified Board Certified cardiologists. If you or your doctor feel that you need to have a cardiology evaluation, every effort will be made to see you promptly. Some urgent (non-emergency) evaluations may be done on the same day if needed.
We respect your time and strive to minimize your waiting time in our office; however, unexpected delays can occur due to emergencies, so please be patient. Your referring physician, if any, will be kept informed of your progress at all stages of your treatment.
As a new patient, you should:
- Bring ALL your medications with you. The actual bottles are better than a written list. This way we can be certain of what you are taking.
- Bring your insurance information
- Complete the New Patient Registration forms, the HIPAA and medical Information forms which may be downloaded here or by going to our "on-line forms page"
- Attempt to bring copies of any relevant medical information or medical records (this is especially true if you are transferring care to us from another cardiologist or if you are self referred). To make this process easier, you may download the release of information and send to your physician so they can send us your records. Again, this is important if you have been seen by another cardiologist in the past.
- Arrive 30 minutes prior to your appointment to allow time for registration.
